The Prenatal Registry, Information and Resource Referral Center is available to all women in our community who are pregnant or thinking of becoming pregnant, and their families.
We welcome your calls to the office. We are out in the community too and can meet you at your home or another place. The Prenatal Registry questionnaire is expected to be filled out by all pregnant women. Once the questionnaire is completed it needs to be deposited in drop-boxes available at the Phoenix and Victoria Labs, the Family Practice offices and the Prenatal Registry office or you can always mail it in.
